Phase 03 · Continuing care
Continuity
Consolidation and ongoing support.
As the intensity of treatment reduces, the relationship does not have to end. CLAIREN Continuity offers ongoing psychotherapy with Sander Tubbergen, space to consolidate changes and support with the difficulties that still need attention.
Sessions are usually less frequent, with a clear plan for contact between appointments. Clients moving from Active Treatment keep full access to CLAIREN Connect throughout their agreed Continuity care.
- Sessions
- At an agreed interval, commonly fortnightly
- Between sessions
- Full CLAIREN Connect access and defined routine support
- Duration
- Individually agreed and regularly reviewed
- Fee
- From £650 per month
The initial enquiry conversation is free of charge. Continuing care is agreed separately, with no automatic enrolment.
A continuing relationship
Less intensive care. The same personal attention.

Stepping down from Active Treatment means adjusting the level of care, not starting again with someone new. You continue working with Sander, building on the understanding and trust developed together.
Continuity may also suit someone with an established, stable recovery who is new to CLAIREN and wants specialist ongoing support. An appropriate assessment is needed before care begins; recent assessment information can be considered so useful work is not repeated unnecessarily.
Continuity is not a required final phase or a way to prolong treatment. It continues only while it is helpful, clinically appropriate and wanted.
The work
Space for what comes next
Life after intensive treatment can bring new questions as well as familiar pressures. The focus of your sessions develops with you, rather than following a fixed programme.
Consolidating change
Understanding what has helped and how to keep useful changes workable in daily life.
Recognising difficulties early
Reviewing warning signs, setbacks and situations in which old patterns become more compelling.
Relationships and responsibilities
Working with trust, boundaries, intimacy, professional demands and significant life changes.
Deeper psychological work
Exploring underlying emotional and relational patterns where appropriate, within Sander’s clinical scope and with specialist input when needed.
The aim is consolidation and ongoing support, not a promise of a particular outcome. If your needs change, the plan and the level of care are reviewed with you.
How it works
A rhythm that remains useful
Individual sessions are commonly fortnightly, with their length, frequency and format agreed before Continuity begins. We also agree what contact between sessions will be helpful and set a date to review the arrangement.
Agree the next phase
Review your needs, treatment priorities and the support you want to retain. Confirm the session schedule, contact arrangements and fee in writing.
Stay connected
Continue individual therapy, use your client space and bring relevant experiences into an agreed brief weekly check-in.
Review together
Consider whether the work is helping and whether frequency, focus or support should change. There is no automatic increase or extension.
Reviews are part of your agreed sessions, not an extra course of appointments. The next review date is recorded in your care plan, with an earlier review whenever concerns arise.
CLAIREN Connect
Your client space stays with you
If you transfer from Active Treatment to Continuity, you keep full access to CLAIREN Connect, without a downgrade or interruption at the phase change. Your existing plans, resources and shared documents remain available alongside your continuing care.
Full access throughout Continuity
Check-ins and messaging
Use the same private client space for brief check-ins and routine contact with Sander.
Your current plan
Keep your priorities, agreed actions and support arrangements in view.
Session preparation
Note questions and experiences you would like to discuss.
Documents and resources
Access your shared summaries and selected materials as the work develops.
Clients joining CLAIREN directly for Continuity receive full access once their care is agreed, the Continuity Agreement is signed and the agreed initial payment is received. There is no public sign-up.
Full app access means access to all client functions, not unlimited clinician time or continuous monitoring. The contact arrangements below apply throughout Continuity.
Between sessions
Contact with clear boundaries
You can share a brief update or ask a question connected with your care without waiting for the next appointment. This contact supports the therapy; it is not a replacement for a session.
Continuity messaging arrangements
- Check-ins
- One brief check-in each week, where useful, with the day agreed in your care plan. Sander reviews it personally; daily check-ins are not included.
- Support hours
- Monday–Friday, 09:00–17:00, and Saturday, 09:00–12:00, UK time.
- Routine response
- Messages and check-ins are reviewed at scheduled points, not continuously. You receive an acknowledgement or brief reply by the end of the next scheduled support day.
- Weekend example
- A message sent after 12:00 on Saturday receives a routine response by 17:00 on Monday, unless a closure has been agreed in advance.
- Scope
- Brief updates, questions about your agreed plan and session arrangements. Extended discussion, additional appointments and intensive or daily support are not included unless separately agreed.
- Outside support hours
- You may send a message, but should not expect it to be read or answered until the next support period. Holiday and absence arrangements are explained in advance.
If a question needs more time, we acknowledge it and agree whether it belongs in your next session or needs a separate appointment. Any additional routine work and fee are discussed before you commit.
CLAIREN Connect is not live chat, an emergency service or a continuously monitored safety system. If you need urgent medical or mental-health help, use the urgent-care arrangements agreed in your plan rather than waiting for a reply.
Review
The right level of care, not care by default
We review your priorities, wellbeing, experience of the therapeutic relationship and the usefulness of the current support. This includes whether to continue, reduce frequency, change the approach or plan an ending.
If addictive behaviour returns or your circumstances become more difficult, the first step is to consider safety and understand what has changed. We may recommend a different level of treatment or medical or psychiatric input, rather than relying on more app messages.
Any change in routine care or fee is discussed and agreed before it begins. A review does not commit you to additional treatment.
Planning an ending
An ending considered together
A planned ending will be a collaborative decision with you, with two weeks’ notice. We use the final review to consider progress, any remaining needs or risks, and appropriate next steps, including onward support where needed.
You remain free to stop attending; you do not need permission to end therapy. Notice, any fees due, app access and access to your records are explained in your written agreement.

Two weeks’ notice applies to a planned ending. Your written terms explain cancellation, fees during the notice period and how any unused prepaid care is handled; there is no automatic commitment to another treatment phase.
Discuss suitability, the proposed session schedule and the full cost before deciding whether to proceed.
Privacy
Personal care, with clear privacy arrangements
We agree how appointments, messages and documents should be handled. Information is not routinely shared with an employer, colleague, partner or family member without your permission.
Confidentiality and its limits are explained before care begins, including supervision, necessary administrative or technical access, safeguarding, serious risk and legal or professional obligations. Private app access does not mean anonymous care or an absence of clinical records.
When Continuity ends, routine messaging and full app access end on the agreed date. We explain how to obtain your documents and request records; access to existing records is not conditional on buying further care.
Questions
Questions before you begin
Do I have to continue after Active Treatment?
No. Continuity is an option, not a required final phase. We consider whether it would be helpful and agree it separately before it starts.
Do I keep full access to CLAIREN Connect?
Yes. Clients transferring from Active Treatment keep full access to all client functions throughout their agreed Continuity care. Your existing shared plans, resources and documents remain available.
The level of clinical contact changes to the Continuity arrangements. Full app access does not mean unlimited messaging support, daily reviews or continuous availability.
Can I join without having Active Treatment at CLAIREN?
Yes, where continuing care is appropriate to your assessed needs. We first review your circumstances and any useful recent assessment information; further assessment and any separate fee are explained before you commit.
Does from £650 mean two sessions every month?
Not necessarily. Sessions are commonly fortnightly, which is not identical to two sessions each calendar month. Your written agreement sets out the actual session dates or allowance, session length, included support and monthly fee.
The starting price does not imply an unlimited number of appointments or additional support.
Can appointments be remote?
Yes, where clinically appropriate. In-person arrangements depend on availability and any agreed additional costs; sessions from another country need prior review of suitability and applicable professional requirements.
How do I end Continuity?
A planned ending is a collaborative decision with you, with two weeks’ notice and a final review of progress, remaining needs and next steps. You do not need permission to stop attending.
The written agreement explains notice-period fees, unused prepaid care and when app access and routine messaging end.
Consider the next phase, together
If you are thinking about ongoing support, a conversation with Sander can help clarify what would be useful. There is no assumption that you need to continue or that one arrangement will suit everyone.
The initial enquiry conversation is free of charge. Any assessment or continuing care is agreed separately.
Continuity is planned outpatient care, not an emergency or medical detoxification service. Enquiry messages and CLAIREN Connect are not continuously monitored.
